Shoulder pain does not automatically mean a rotator cuff tear. The history and examination help decide whether X-ray, ultrasound, MRI, or no imaging is most useful.

Shoulder pain has a way of reaching into the rest of your day. Sleeping on that side wakes you up. Reaching into a cabinet hurts. Putting on a shirt, fastening a seatbelt, or grabbing something from the back seat suddenly requires a strategy.

People often assume that pain means a torn rotator cuff and that an MRI is the next step. Sometimes a tear is part of the problem. Several other conditions can create a very similar pattern, and many shoulder problems can be evaluated without starting with an MRI.

The history and examination help decide what imaging, if any, is likely to change the plan.

Why can shoulder pain be difficult to identify?

The shoulder gets its motion from several joints working together with the rotator cuff, biceps tendon, capsule, shoulder blade muscles, and the neck. Pain felt in the shoulder may come from one of those structures or be referred from somewhere else.

The location is helpful, but it isn't a diagnosis by itself. Pain along the outside of the upper arm can occur with rotator cuff irritation, bursitis, a tear, or other conditions. Pain near the shoulder blade may relate to the neck, muscle overload, or altered shoulder mechanics.

The way the pain behaves usually tells us more.

Common patterns we see

The rotator cuff is a group of muscles and tendons that helps lift and rotate the arm. Irritation around these tendons and the nearby bursa often causes pain at the front or side of the shoulder, especially with overhead reach, lowering the arm, or sleeping on the painful side.[1]

Some people retain good strength but hurt through part of the motion. Others develop weakness or stop using the arm enough that it becomes deconditioned.

Rotator cuff tear

A tear may happen suddenly after a fall or forceful lift. It can also develop gradually as a tendon changes with age and repeated use. Sudden pain, a pop, and immediate weakness after an injury deserve timely assessment.[2]

Not every tear seen on a scan explains the symptoms. Degenerative tears can be present without pain. The examination helps connect the imaging finding with what the shoulder can and cannot do.

Frozen shoulder

Frozen shoulder, also called adhesive capsulitis, usually causes progressive pain and a marked loss of motion. Reaching overhead and behind the back becomes difficult. The key clue is that the shoulder is limited when you move it yourself and when someone else carefully tries to move it.[3]

It is more common in people with diabetes and certain thyroid conditions, and it may appear after a period of immobilization. Imaging can help exclude other problems, but frozen shoulder is often recognized through the history and examination.

Arthritis

Shoulder arthritis can cause deep pain, stiffness, grinding, and loss of motion. Symptoms may build slowly. X-rays are usually more useful than ultrasound for showing joint-space narrowing, bone spurs, and alignment.

Biceps or labral pain

The long head of the biceps tendon and the labrum can contribute to pain in the front or deep inside the shoulder. Catching, painful clicking, a feeling of instability, or symptoms during throwing may raise suspicion, but these experiences overlap with other conditions.[4]

Pain referred from the neck

Neck problems can send pain toward the shoulder or down the arm. Numbness, tingling, pain below the elbow, symptoms changed by neck position, or weakness in a nerve pattern make the neck examination especially important.

Pain from the chest, lungs, or abdomen can occasionally be felt near the shoulder. That possibility matters when shoulder discomfort occurs with chest pressure, shortness of breath, sweating, faintness, abdominal pain, or illness.

What happens during a shoulder evaluation?

The clinician will usually ask how the pain began, where it travels, whether there was an injury, which motions hurt, and whether sleep is affected. Your work, exercise, throwing, swimming, tennis, golf, and strength-training habits can provide useful clues.

The examination may include:

  • neck motion and a brief nerve assessment.
  • shoulder-blade position and control.
  • active and passive shoulder motion.
  • rotator cuff and arm strength.
  • tenderness around the joint, biceps tendon, and acromioclavicular joint.
  • selected movements that load the suspected tissues.
  • comparison with the other shoulder when useful.

No single maneuver proves a diagnosis. We look for a pattern across the history, motion, strength, and symptom response.

When is an X-ray helpful?

For chronic shoulder pain, the American College of Radiology generally recommends shoulder radiographs as the initial imaging study.[5] X-rays can show arthritis, fractures, alignment, calcification, and changes around the joint.

They don't show every tendon or labral problem. A normal X-ray doesn't mean the pain isn't real. It simply answers a different set of questions.

After a significant fall or other acute injury, X-rays are also generally the appropriate first study because a fracture or dislocation must be considered.[6]

When can ultrasound help?

Musculoskeletal ultrasound provides a real-time view of selected tendons, the biceps tendon, the bursa, and fluid around the shoulder. The examiner can watch structures move as the arm changes position.

When chronic shoulder pain suggests a rotator cuff disorder and initial X-rays are normal or inconclusive, the ACR considers ultrasound or MRI appropriate next studies.[5]

Ultrasound may help evaluate:

  • rotator cuff tendinopathy and some partial or full-thickness tears.
  • subacromial-subdeltoid bursitis.
  • the long head of the biceps tendon.
  • calcific deposits.
  • joint or bursal fluid.
  • a specific target when an image-guided procedure is being considered.

The quality of the examination depends on training, equipment, and the clinical question. Ultrasound does not provide a complete view of every deep structure in the shoulder.

When might MRI be the better test?

MRI provides a broader view of the rotator cuff, muscles, bone marrow, cartilage, labrum, and other internal structures. It may be useful when there is substantial weakness after an injury, concern for a tear that could need surgical repair, suspected instability or labral injury, unexplained symptoms, or a treatment decision that depends on more complete anatomy.

An MRI often isn't necessary before beginning treatment for an uncomplicated, nontraumatic shoulder-pain pattern. Scans commonly find age-related changes that may not be the source of pain. Ordering a detailed test before defining the clinical question can create more confusion than clarity.

Does an imaging result decide the treatment?

No. A tendon abnormality on ultrasound or MRI doesn't automatically require an injection or surgery.

Many shoulder problems improve with a plan that restores motion, adjusts painful loads, and gradually rebuilds strength. Treatment may include targeted rehabilitation, temporary changes to work or training, medication when medically appropriate, sleep-position changes, or a home program.

An injection may be considered for a selected diagnosis when the expected benefit, risks, alternatives, and timing make sense. A procedure should support a broader recovery plan. It shouldn't replace an examination or a thoughtful rehabilitation strategy.

Surgical consultation may be more urgent after an acute traumatic tear with marked weakness, a fracture or dislocation, recurrent instability, or another problem that is unlikely to recover with routine conservative care.

When should shoulder pain be assessed promptly?

Seek urgent evaluation after an injury if the shoulder looks deformed, you cannot move the arm, pain is severe, the arm becomes numb or cold, or you suspect a dislocation. A shoulder dislocation is an emergency injury and should not be forced back into place without appropriate medical care.[7]

Prompt assessment also makes sense for:

  • sudden weakness after a fall or forceful lift.
  • fever, redness, warmth, or major swelling around the joint.
  • rapidly worsening pain.
  • new numbness or progressive arm weakness.
  • shoulder discomfort with chest pressure, shortness of breath, sweating, faintness, or other concerning symptoms.

If symptoms may represent a medical emergency, call 911 or seek emergency care.

How to prepare for an appointment

Bring any prior imaging reports and a current medication list. Wear clothing that allows the neck, shoulder, and upper arm to be examined.

It helps to note:

  • the motion that hurts most.
  • whether pain travels below the elbow.
  • whether you have numbness, tingling, or weakness.
  • how the problem affects sleep.
  • the exact activity or injury that started it.
  • what treatments you have tried and how the shoulder responded.

If a specific exercise or sports motion causes the problem, a short video may help when it cannot be reproduced safely in the office.

What should you take away?

Shoulder pain doesn't automatically mean a rotator cuff tear, and a suspected tear doesn't automatically mean surgery. The pattern of pain, motion, strength, and injury history helps narrow the possibilities.

X-ray, ultrasound, and MRI answer different questions. A useful evaluation chooses the test that is most likely to change what happens next.

Do I need an MRI for shoulder pain?+
Often, no. The history, examination, and sometimes an X-ray can determine the initial plan. MRI is more useful when detailed anatomy will affect treatment.
Can ultrasound diagnose a rotator cuff tear?+
Ultrasound can identify many rotator cuff tears when performed by a trained clinician or sonographer. Accuracy depends on the examiner, equipment, and tear type.
Why does my shoulder hurt at night?+
Night pain can occur with rotator cuff related pain, bursitis, arthritis, frozen shoulder, and other conditions. The examination helps narrow the cause.
Does clicking mean I tore something?+
Not necessarily. Clicking can occur with normal movement, tendon motion, arthritis, labral problems, or other conditions. Painful catching, weakness, or instability deserves assessment.
Will an injection fix shoulder pain?+
An injection may help selected conditions, but it does not fit every diagnosis and cannot replace rehabilitation when strength, motion, or load tolerance needs attention.
